"The Individual Needs" Subacute Care, or transitional care, is generally provided after a hospitalization when the individual does not need the acute care setting of a hospital but is not ready to go home because they still require medical treatment or therapy. Nesconset Nursing Center's highly trained staff of health care professionals includes rehabilitation therapists, registered and licensed practical nurses and certified nurses assistants. This specialized staff's goal is to facilitate your recovery and promote independence. Patient education is emphasized because of its importance in preparing you for your return home. Because of our success in this area, Nesconset Nursing Center has contracted with many insurance companies and HMOs to provide subacute care for covered members.
