Nesconset Nursing Center's PARTNERS program was developed in 1991
to meet the needs of those individuals who require short-term rehabilitation prior to returning
to the community. This might include, among others, those affected with fractures, amputations, strokes,
brain and spinal cord injuries, total hip and knee replacement and multiple trauma.
A team of rehabilitation professionals develop an individualzed plan of care to restore each Rehab
Resident to their maximum level of function and independence.
